  • The BL Gym Body Membership entitles the subscriber to monthly services outlined in the most current BL Gym Body Membership Program current to the date of this contract. All medical services and products offered must be approved by clinical staff as appropriate and applicable before services/products offered.
  • hereby agree to all terms and conditions outlined in this Program Subscription Service Contract entered into with Aesthetic Medical Management (“DBA” Beauty Lounge Med Spa) located at 2475 Ventura Blvd, Camarillo, CA 93010,
  • _________ PRICES AND TERMS: You agree to pay the monthly subscription price of $250 US dollars per month for 12 consecutive months. At the time of account creation and purchase, you need to provide accurate information regarding your account, including a valid credit card, and you must certify that you are over 18 years of age and legally able to enter into a valid and binding agreement with Aesthetic Medical Management ( “DBA” Beauty Lounge Med Spa).
  • __________ ONE TIME PAYMENT: You have the option of paying the full 12 month price of the monthly subscriptions upfront for the price of $3,000 US Dollars.
  • __________ MONTHLY RECURRING PAYMENTS: Standard monthly subscription is a monthly recurring payment that will take effect on same date each month as determined by date of the first payment. All monthly subscriptions renew automatically for 12 consecutive months then expire until new subscription enrollment by the subscriber. The subscription will automatically renew every 30 days, and your credit card will be charged at this time unless you cancel the subscription. See “Terms of Cancellation
  • __________ NO REFUNDS: Except if otherwise expressly stated, Aesthetics Medical Management (DBA Beauty Lounge Med Spa) will not refund any subscriptions. No refund will be given for any unclaimed services or products past the current paid annual membership period.
  • __________ DISPUTE OF CREDIT CARD CHARGES: If you have a disagreement regarding a credit card charge, you may contact the Aesthetics Medical Management ( DBA Beauty Lounge Med Spa) office manager in writing within 90 days of the date of the initial credit card charge. You agree that Aethetics Medical Management ( DBA Beauty Lounge Med Spa) is not liable for any charges or errors not reported within such 90-day time frame; and you expressly waive any right to dispute a charge to your credit card if you have not contacted Aesthetics Medical Management (DBA Beauty Lounge Med Spa) in writing with respect to any disputed charge within such period.
  • __________ TERMS OF CANCELLATION: You may terminate your subscription agreement under all of the following terms only:
    1. Must submit your request in writing within 30 days prior to next monthly charge.
    2. The effective date of such termination will be the end of the current billing period.
    3. Any remaining services or coupons must be surrendered at the time of cancellation
    4. Subscriber will be charged the balance of services used at the noted retail pricing minus the months paid to the program.
    5. You shall not be entitled to any refund as a result of the termination of your subscription to any Product/Services under this Agreement.
    6. We may terminate your right to use and/or access any future membership services/products at any time is our sole discretion, with or without prior notice.
  • _________ RESTRICTIONS This agreement is non-transferable. All members must medically qualify for services to be provided. You agree that you will not:(a) Sell, distribute, license, rent, or otherwise exploit any Services/Product provided, or any element of it, for any commercial purpose (b) Make any Service/Product, or any element of it, available to other persons. All services must be claimed by the end of the 12 month membership period. Any unused services will be forfeited at the end of the 12 month membership period.
    By signing below, I acknowledge that I have read the above contract agreement which constitute the “Subscription Service Agreement and Contract” as specified above and that I have read, understand, and accept all of the terms, conditions as outlined in therein.
